At Tuomey, we know home is where the heart is. We understand that in some cases, home care is a viable and beneficial option for a patient. We offer our patients a variety of services that can be provided in-home.
In-home Services
Hospice Care
Through this in-home program, Tuomey nurses trained in compassionate care for people in the last phases of incurable disease help patients live as fully and comfortably as possible. Tuomey’s Hospice nurses have specialized training in pain management and symptom control, and can provide these services in the comfort of the patient’s home.

Rehab Services
Our therapists provide in-home physical therapy and occupational therapy to patients with a variety of rehabilitative issues such as orthopedic problems, cardiac problems, injuries sustained from trauma, or conditions associated with neurological disease.

Home Health and Nursing Care
Through Tuomey Home Health, patients can receive care from a registered nurse, physical therapist, occupational therapist, social worker, home health aide or dietitian, according to their physician-directed plan of care. The goal of the service is to meet the unique healthcare needs of each patient and their family using a holistic approach. Services performed in the home range from wound care and pain management to administering IV antibiotics.

Bridge Program
Persons who have a life-threatening illness face a dilemma. Despite being aware of the gravity of their illness, they may seek active, aggressive treatement for their disease. They need skilled care for their treatments and extra emotional and social support for themselves and their families. Tuomey Home Health's Bridge Program brings together quality home health care in a Hospice-like environment to persons with a life expectancy of one year or less. The Bridge Program uses the same specially trained staff as the Hospice program, assuring continuity of care should it become appropriate to transfer the patient to Hospice.
